Every employee on the Argos medical plan MUST complete the Tobacco Affidavit, attesting that you are Tobacco Free and/or complete the Navigate Tobacco Cessation Program. Failure to do so will result in the employee paying a $50 monthly surcharge, totaling $600 next year.

You can find the Tobacco Affidavit on the home screen once you login to Argos Live Well.

Click the words, “Tobacco Affidavit” then attest:

• “I am not a tobacco user” and submit your affidavit. By completing this task, you no longer will be subjected to paying the $50 monthly surcharge next year.

OR

•  Go to the Challenges tab and then click the Video Courses tab.
• Click the Join button on the 3rd course, the Tobacco Cessation Course to get started. (As long as you watch all the required videos before November 30th, you will complete the course on time)

Then go back to the Tobacco Affidavit button and attest that, “I am a tobacco user and attest that I have completed the Tobacco Cessation Video Learning Course”. By completing this task, you no longer will be subjected to paying the $50 monthly surcharge, totaling $600 next year.
