Form 1095-C provides information about the health coverage offered by your employer and about whether you enrolled in this coverage. These forms will be mailed but they are available right now on SmartBen. The forms are listed under the ACA Statement link populated on the employee’s homepage. 

Although you may use the information on the form to help complete your tax return, these forms should not be attached to your return or sent to the IRS.
